【DSD】Those who must bring a doctor's note
DSD/Licence holders: for your own safety, all representatives must have ALL participants read THOROUGHLY this medical questionnaire before confirming your booking 【DSD Medical Checklist】 （←CLICK）Any item with an answer "YES" means you must bring a doctor's note indicating "I find no medical conditions that I consider incompatible with scuba diving." and signed and dated by the doctor with the clinic's details or letter head. Please also indicate any medical conditions WHEN you are booking (fill in Remarks).
Those with asthma or those with diabetes requiring insulin injections may not join our activites.

| 70~80 | Ocean Entry Diving to the Blue Cave while swimming with tropical fish. For about 100m. Arrival at the Blue Cave. Back to entry point |

| 150 | Shuttle Back to our shop. Completion. Lightly shower. Change back into your own clothes. Returning wetsuits. |
